State of Nevada Electronic Livescan Fingerprint Submission Forms:
- State of Nevada Private Investigator License Board (PILB) Fingerprint Form: PILB_FingerprintForm. PILB applicants must register and pay the license fee before they may be fingerprinted.
- NV PILB Work Card Application Fingerprinting Express is an approved vendor for the PILB and we can provide the photos required for licensing as well.
- Nevada Manufactured Housing Division: Nevada Manufactured Housing Division Fingerprint Livescan Form
- Nevada Gaming Control Board (NGCB) Employee Registration Link. Use this link if you are not paying the license fee through your Human Resources Department: Nevada Gaming Control Board Fingerprints Staff must register online and pay the license fee online before they may be fingerprinted. A fingerprint receipt will be emailed to you. If you cannot bring in the emailed fingerprint receipt we are happy to print it for you. Fingerprinting Express is an approved vendor for the Nevada Gaming Control Board.
Mailing in FD258 fingerprint cards?
Fingerprint Background Check Applications for hard cards. One is mailed to the State of Nevada (for out of state fingerprint cards please check with that state) and one is mailed to the FBI:

*FBI FD-258 Fingerprint Card (in stock at Fingerprinting Express)
*Money Order made payable to Treasurer of the United States
